Telecare's program in Santa Clara County is a 100-member high-fidelity Assertive Community Treatment (ACT) program for adults and older adults with severe and persistent mental illness (SPMI), co-occurring substance use issues, and / or other challenging and complex needs that increase their risk for poor treatment outcomes. Wraparound services and supports will not only include clinical interventions, such as multidisciplinary assessment and treatment, team-based case management, 24 / 7 / 365 crisis intervention, medication support, and integrated co-occurring substance abuse services, but also non-mental health support in such areas as community integration, housing, employment and education, transportation, and benefits.
What You Will Do to Change Lives
Under direct supervision of the Clinical Director / Administrator, this entry-level licensed management position supervises multidisciplinary teams, coordinates service needs, and collaborates with other services and agencies. Completes assessments and determines the appropriate level of services for new members. Employees in this role are required to participate in a structured on-call rotation, remaining accessible and prepared to respond to work-related needs within a designated timeframe. Rotations are scheduled in advance. While on-call, employees must actively monitor communications (e.g., phone, email) and respond in accordance with operational protocols.
Shifts Available :
Full-Time; 8 : 00 AM - 5 : 30 PM | Days : Monday - Friday
Expected starting wage range is $110,112.91 - $136,029.35. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
QUALIFICATIONS
Required :
Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
Willingness to use your personal vehicle to drive clients to appointments and groups, etc. (weekly mileage reimbursement at the IRS rate)
Willingness to use your personal vehicle to attend meetings, etc. (weekly mileage reimbursement at the IRS rate)

ESSENTIAL FUNCTIONS
Arizona Programs :
o Practices psychotherapy for the purpose of assessment, diagnosis and treatment of individuals, couples, families and groups specifically related to license (social work, counseling, marriage and family, or substance abuse)
