This is a great job opportunity for an experienced Director of Shared Savings or Director of Care Coordination with experience managing Medicare Chronic Care Management and MHN Accountable Care Organization Program.
Your Goal :
This position is accountable for and works to ensure maximization of shared savings and quality performance payments and oversight of other care navigation and care management programs.
How You'll Help Patients :
- Provides all forms of leadership, supervision, direction, training and role modeling to all subordinate staff, including the developing and leading of staff meetings and regular individual supervisions.
- Promote and maintain a professional work environment
- Ensure applicable care management screenings are completed and properly documented.
- Implements, manages and promotes continuous monthly patient monitoring to assess the efficacy of the care plan for patients.
- Oversee Medicare Chronic Care Management and MHN Accountable Care Organization Program.
- Ensure patients are provided with education regarding labs, procedures, and disease processes and any other health education information deemed necessary.
- Serve as the liaison for quality incentive and shared savings matters between Greater Family Health and third-party payers and / or Accountable Care Organization(s).
- Responsible for overall administration of IDHS Better Birth Outcomes Comprehensive (BBO-C) Program.
- Implement and maintain BBO-C program across funded service area.
- Reports to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor.
- Engage in all offered training, consultations, and supervisions made available. Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers.
- Perform other duties as assigned.

What You'll Need :
Bachelor's degree in health care or public health related field. Master's degree is preferred.Preferred 3-5 years' experience in health care industry with emphasis in shared savings / quality care management and / or accountable care organizations.Computer literate and has ability to utilize various software, word processing, and spreadsheet programs.Strong knowledge of Healthcare Effectiveness Data and Information Set (HEDIS).Strong knowledge of Medicare Clinical Quality Measures (CQM).
