LCSW / LPC - SUD Therapist for Primary Care : Conyngham Primary Health Care Center / FQHC Wilkes-Barre

Purpose of this Position :

The LCSW / LPC Substance Use Disorder Therapist provides services that include, diagnostic bio-psycho-

social assessments, level of care assessments, American Society of Addiction Medicine placement

summaries, individual and group outpatient substance use disorder therapy, trauma exposure screenings,

domestic violence screenings, medication assessments and medication monitoring that fall under the

Conyngham Primary Health Care Center. This position will serve adults, adolescents and their families.

Responsibilities include, but are not limited to :

• Skilled in motivational interviewing and cognitive behavioral therapy
• Knowledgeable of supportive resources in Luzerne and Wyoming Counties
• Ability to maintain a patient centered approach
• Application of trauma informed care
• Current knowledge of drug trends and their effects
• Being well organized and adaptive within the work environment
• Provide translation services for Spanish speaking clients
• Provide substance abuse therapy to Spanish speaking clients

Qualifications :

Master’s Degree plus Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC)
