Description :
The physical therapist will accurately assess, differentially diagnose, and identify deficits in a patient's ability to move and perform functional activities in their daily lives.
They develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability.
The therapist works with the individual to prevent the loss of mobility.
Essential functions and responsibilities include performing patient evaluations through the process of obtaining history, performing relevant systems reviews, and selecting and administering specific tests and measures to obtain data.
Establishing an individualized plan of care and selecting treatment techniques that address the specific impairments identified in the evaluation is critical, in accordance with physician orders.
The physical therapist conducts ongoing reassessment of the patient and their response to treatment.
Initiating and managing the discharge plan begins at the first visit and continues throughout treatment.
Supervision and delegation to ancillary staff and students must be consistent with state laws and department policy.
Completion of legible, accurate, and clear documentation within the established timeframe identified in department policy and procedure, as well as in accordance with accreditation requirements and insurance regulations, is required.
Communication with the patient, family, caregivers, significant others, and members of the health care team promotes the maximum benefit of care utilizing AIDET.
Assessing the educational needs of the patient, family, caregiver, or significant other and providing education to meet those needs is essential.
Participation in professional growth and development occurs through involvement in educational programs, including in-service meetings, workshops, and by reading current literature concerning the scope of practice.
Actively participating in quality improvement activities is expected.
Providing an environment conducive to safety for patients, visitors, and staff is a priority.
Compliance with appropriate and approved safety standards is mandatory.
The physical therapist must function within the scope of practice as defined by the state’s practice act.
Other related duties as required and directed may be assigned.
