Job Description :

Sunbelt Staffing is seeking an experienced Home Health RN for a contract position in Lake Zurich, IL. The RN will conduct skilled nursing visits, complete required OASIS documentation, and manage patient care using Homecare Homebase (HCHB). This position is ideal for nurses who thrive in independent fieldwork while ensuring excellent outcomes for their patients.

Key Responsibilities :

Perform comprehensive assessments, admissions, recerts, discharges, and resumptions using OASIS

Deliver compassionate, evidence-based care in the home environment

Document visits thoroughly in Homecare Homebase (HCHB)

Collaborate with physicians and team members for individualized care plans

Adhere to agency guidelines and Joint Commission compliance standards

Qualifications :

Current RN license in Illinois

At least 2 years of RN experience, including 1 year in home health

Proficiency in OASIS and Homecare Homebase (HCHB) required

Reliable transportation with valid drivers license and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
