About the Organization

We are a payer organization specializing in managed care, with a strong focus on public insurance programs including Medicare and Medicaid . With over 15 years of partnership and exponential growth in the last 5 years, we are expanding our support for the MyCare Ohio program—a coordinated care initiative for individuals eligible for both Medicare and Medicaid.

Position Overview

As a Care Manager , you will play a vital role in supporting dual-eligible members through a combination of remote and field-based care coordination. You will be the advocate and liaison for members, ensuring they receive comprehensive, compassionate, and community-integrated healthcare support.

Key Responsibilities

Remote (50%)

• Conduct telephonic outreach to members
• Develop and manage individualized care plans
• Coordinate medical appointments and community health services
• Provide ongoing support and education to members and their families

Field-Based (50%)
