A social services organization in New York is currently seeking two APS Case Managers to join their team. These positions involve 100% fieldwork across the Bronx and Manhattan boroughs. Ideal candidates will have experience in case management and a passion for community support.

About the Opportunity :

  • Hybrid or Remote : 100% Fieldwork
  • Shift : Day
  • Schedule : Monday-Friday
  • Hours : 9 : 00 AM-5 : 00 PM, 35-hour work week
  • Setting : Temp-to-Perm (6 months temp, then conversion)

Responsibilities :

  • Providing direct client services, including intake and assessment.
  • Managing a caseload and documenting progress in APSNET and ETO.
  • Monitoring risk and adapting service plans as needed.
  • Referring clients to appropriate external services.
  • Conducting home visits and telephonic communication weekly.

    • Qualifications :

  • Bachelor's degree in Social Work or related field.
  • Minimum of 1 year of case management experience; 4 years preferred.
  • Enrollment in MSW program is a plus.
  • English proficiency; bilingual in English / Spanish is a plus.
  • Strong communication, time management, and organizational skills.
  • Proficiency in Microsoft Office and standard business technology.
  • Physical ability to conduct fieldwork and lift / move up to 10 lbs.
