Wound Nurse LPN

Schedule : Tue, Wed, Thu, Sat, Sun (off Mon & Fri)

Reports To : Director of Nursing / ADON

Position Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to serve as our Treatment Nurse (Wound Nurse) . In this role, you’ll lead our skin integrity and wound care program, provide direct treatments, and support residents in achieving the best healing outcomes.

What You’ll Do

Perform wound and skin assessments, obtain treatment orders, and administer prescribed treatments.

Lead weekly wound rounds, including measurements, care plan updates, and documentation.

Monitor wound healing progress and communicate with physicians on necessary changes.

Educate staff on skin care protocols, wound prevention, and proper treatment techniques.

Ensure infection control procedures and state regulations are followed.

Provide direct resident care and maintain accurate documentation for QAPI and compliance.

Collaborate with nursing staff, physicians, and families to support positive resident outcomes.

Qualifications

Graduate of an accredited Practical Nursing program; current LPN license in the state of practice (required) .

Minimum 1 year of wound / treatment experience in long-term care (preferred).

2+ years of LPN experience in a Skilled Nursing Facility (preferred).

Current BLS / CPR certification.

Strong clinical, communication, and leadership skills.
