About the Role :

The Home Health Field Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality nursing care to patients within their homes, ensuring comfort, safety, and effective treatment outside of traditional clinical settings. This position requires the LPN to assess patient health status, administer medications, and provide wound care while closely monitoring patient progress and reporting changes to the supervising healthcare team. The LPN will collaborate with physicians, registered nurses, therapists, and family members to develop and implement individualized care plans tailored to each patient's unique needs. By working in the patient's home environment, the LPN helps promote independence and supports recovery or chronic condition management in a compassionate and respectful manner. Ultimately, this role contributes to improving patient outcomes and enhancing the overall quality of life for individuals receiving home health services in the Claremore community.

As a Home Health LVN, you will :

Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.

Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.

Applies concepts of infection control and standard precautions in coordinating / performing patient care activities to prevent contamination and transmission of disease.

Assure the continuity of care through delivery of quality patient care.

Provide effective communication to patient / family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.

Monitor assigned cases to ensure compliance with requirements of third party payor.

Promote Agency philosophy and administrative policies to ensure quality of care.

Requirements

Current LVN License

Valid Driver’s License

Valid Auto Insurance

CPR Certification

HomeCare HomeBase experience preferred
