Position Overview
The Clinical Office Coordinator provides administrative and clinical support to ensure the smooth operation of the hospice office. This individual will handle traditional office coordination duties while also leveraging their LPN license to perform clinical administrative tasks including writing physician orders, managing documentation, and supporting compliance and quality efforts.
This is an ideal position for a detail-oriented, organized LPN who thrives in a collaborative environment and enjoys combining clinical knowledge with administrative leadership.
Key Responsibilities
Administrative Coordination
- Oversee day-to-day office operations, ensuring efficiency and organization in all administrative functions.
- Manage incoming calls, correspondence, and documentation for patient admissions, discharges, and care updates.
- Maintain accurate and timely records in the Electronic Medical Record (EMR) system.
- Coordinate schedules, meetings, and communications for the clinical and administrative teams .
- Serve as the first point of contact for internal and external communication, ensuring professionalism and confidentiality.
Clinical Administrative Duties
Utilize LPN licensure to review and transcribe physicians orders under direction of the Director of Nursing or Medical Director.Ensure orders are accurate, signed, and entered promptly to support timely patient care.Verify medication lists, plan of care updates, and clinical documentation for completeness and compliance.Assist with maintaining and auditing medical records in accordance with Medicare, state, and hospice regulations .Support coordination of patient visits, hospice supplies, and clinical workflows.Act as a liaison between field staff, providers, and the hospice administrative team.Quality & Compliance
Participate in Quality Assurance and Performance Improvement (QAPI) activities.Help ensure timely submission of required documentation and reports.Maintain awareness of compliance and policy standards for hospice operations.Qualifications
Active LPN License in the state of Arizona (required).Minimum 2 years of healthcare office or clinical coordination experience (hospice or home health preferred).Strong understanding of hospice regulations and documentation standards .Proficiency in EMR systems and general office software (Microsoft Office, Google Workspace, etc.).Excellent organizational, multitasking, and communication skills.Compassionate demeanor and ability to handle sensitive patient information with integrity.Why Join Uplift Hospice
