Your Role as an LPN

As an LPN in our Home Health & Hospice division , you will work under the guidance of a Registered Nurse (RN) to ensure patients receive the best care possible. Your responsibilities include :

• Administering nursing treatments and medications as prescribed
• Assisting with activities of daily living and pain management
• Educating patients and caregivers on self-care techniques
• Monitoring patient conditions and reporting changes to the RN or physician
• Participating in interdisciplinary team meetings for comprehensive care planning
• Ensuring accurate documentation and availability of necessary supplies
