Job Opportunity : Director of Social Services

Schedule : 40 hours

Key Responsibilities :

• Responsible for implementing and documenting a plan of care related to the resident’s social / emotional needs in the electronic medical record.
• Responsible for collaborating with the nursing, MDS and rehabilitation departments to facilitate discharge plans for residents.
• Informing and educating the staff / resident / family / responsible party of the resident’s personal and property rights, as well as his / her rights related to health services as pertaining to the resident.
• Maintains updated resources for residents and families in regards to community resources and services, as well as service agencies and organizations. Maintains a record of all reported concerns, assists in resolving them and assures appropriate follow up and documentation.
• Easily accessible to residents, family and staff. Provides timely referrals to appropriate agencies for assistance to the resident / family (i.e. conservatorship application, Medicaid application, etc).
