Registered Nurse (RN) Clinical Manager / Director of Nursing

We are seeking an experienced and compassionate RN / DON Clinical Manager with a strong background in home health to join our leadership team. This individual will play a pivotal role in overseeing clinical operations, ensuring the highest quality of patient care and maintaining regulatory compliance within our home health agency. The RN / DON Clinical Manager will serve as a mentor and leader to the clinical staff, driving performance, patient outcomes, and operational excellence.

The Clinical Manager coordinates and oversees all direct care patient services provided by clinical personnel :

• Develops, plans, implements, analyzes, and organizes clinical operations.
• Supervise and support clinical staff including RNs, LPNs, therapists, aides, and other home health professionals.
• Review and approve patient assessments, care plans, and documentation.
• Conduct routine case conferences and participate in interdisciplinary team meetings.
• Monitor clinical performance, caseloads, and patient outcomes.
• Provide field supervision and performance evaluations as needed.
• Collaborate with the intake, scheduling, and administrative teams to ensure efficient operations.
• Assist in hiring, onboarding, and training new clinical staff.
• Participate in quality improvement initiatives and ensure documentation accuracy.
• Serve as a liaison between clinical staff, physicians, patients, and families.
