COE Program Manager

Our Centers of Excellence (COE) Care Management Team plays a critical role in helping patients overcome barriers, connect with vital services, and find hope in recovery. As the COE Program Manager, you will lead a passionate team that brings peer recovery support and care coordination directly into communitiesbuilding bridges to recovery and empowering lives.
The Role :
The COE Program Manager oversees the day-to-day operations of our Centers of Excellence program. In this leadership role, you will guide, train, and support a team of Certified Recovery Specialists and Care Coordinators while promoting the integration of recovery support services into the patient experience across CleanSlate centers. This is a dynamic, hands-on role ideal for someone who thrives in collaborative environments and is committed to advancing recovery services in behavioral healthcare.
What You'll Do :
- Manage the COE Care Management Team, providing regular supervision, guidance, and development opportunities
- Ensure seamless coordination between patients, staff, and community partners for effective recovery support
- Facilitate team learning through quarterly COE collaboratives and customized training sessions
- Monitor and audit documentation and program data to ensure compliance and drive quality improvement
- Serve as a liaison between COE staff, Center Managers, and Center Medical Directors
- Support integration of behavioral health and recovery support services into all aspects of patient care
- Provide on-call support as needed to Certified Recovery Specialists
- Champion a culture of shared learning, collaboration, and continuous improvement
- Maintain alignment with regulatory standards, payer expectations, and organizational protocols
What We're Looking For :
Bachelor's degree in Human Services or a related field (Chemical Dependency, Social Work, Counseling, Psychology preferred)3+ years of experience in a human services or substance use treatment setting, including supervision and program planningActive Certified Recovery Specialist Supervisor (CRSS) Certification requiredKnowledge of community resources, referral networks, and peer support integrationExperience in case management or care coordination strongly preferredExcellent communication, organizational, and leadership skillsAbility to manage multiple priorities, provide clear supervision, and work independentlyValid driver's license and ability to travel between sites as neededWhy CleanSlate?
