Pride Health is hiring a dietitian for our client in Sioux City, IA! This position is a PACE program, which is a unique model of care in the U.S. designed to help older adults who would otherwise need nursing home care but want to stay in their homes and communities as long as possible.

What You’ll Do
- Assess nutritional needs by reviewing medical history, lab results, medications, and diet orders.
- Develop and implement individualized nutrition care plans.
- Provide education on diet, food safety, and sanitation to participants, families, and staff.
- Document participant information and care plans in the electronic medical record (EMR).
- Actively participate in daily Interdisciplinary Team meetings and care coordination.
- Maintain knowledge of PACE program regulations, compliance, and best practices.
Requirements :
Bachelor of Science in Dietetics / Food & Nutrition (meeting ADA requirements).Completion of internship or coordinated undergraduate program (ADA approved).Registered Dietitian (or registration eligible) with the Commission on Dietetic Registration.Licensed (or license eligible) with the State of Iowa.Current CPR / BCLS certification.Adult Mandatory Reporter certification.ServSafe (or ANSI-accredited food safety certification) within 90 days of hire.Valid driver’s license & auto insurance.Skills & Abilities
Strong communication skills for working with participants, families, and healthcare teams.Ability to adapt to changing priorities in a fast-paced care setting.Organizational and documentation skills, including proficiency with Microsoft Office.Apply today!
