job_description.job_card.job_descriptionHome Health setting covering Inland Empire, CA and surrounding regions Day shifts with 5x8 hour schedule, typically 8 : 00–16 : 30 1 : 1 patient ratio with up to 6 patients per day and 30 productivity points standard One weekend shift per contract required No floating between units No on-call requirement Travel pairs are accepted, but each traveler is assigned to their own territory Clients are grouped to minimize drive time, though a maximum radius of 50+ miles may be occasionally necessary Primary duties include patient evaluation and treatment in a home health environment Mileage is reimbursed at the IRS standard rate Standard business or casual attire; no specific scrub color required Parking is free at this assignment Must be comfortable working independently in the field First-time travelers will not be considered for this position Former permanent employees must have a minimum 6-month separation and HR approval before eligibility as a traveler Active PT license required (license required if applicable) Minimum 1 year of recent, relevant skilled home health PT experience BCLS certification required Oasis experience required for staff completing Start of Care visits Proficiency in HomeCare HomeBase (HCHB) preferred Active driver's license and reliable personal transportation required Timely communication and documentation skills required Completion of a Hospice / Home Health Checklist is mandatory RTO must be submitted at time of application Must provide 2 professional references from a supervisor within the past 2 years High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
