Job Highlights :

We are looking for a compassionate Home Health LPN for our Reedsburg location.

Nurse's cover territory of Sauk, Baraboo, Wisconsin Dells, Adams / Friendship, Lyndon Station, Mauston, New Lisbon, Richland Center, Muscada, Lavalle, Loganville, Reedsburg.

• Whole territory will only be covered when on call.
• Areas will be assigned.
• 5 regular visits in a 8 hour day.

Schedule : M-F 8-4 : 30.

PRN requirements : 2 shifts / month. 1 weekend and 1 Holiday

Job Summary :

Provides care to patients under the direction of a registered nurse or physician, functioning within the scope of license.
