Nurse Practitioner NP

RiteMed Group is seeking a dedicated and skilled Nurse Practitioner to provide on-site care at two skilled nursing facilities : ExcelCare at Egg Harbor and The Pines at Atlantic City. This is a full-time opportunity with flexibility and support from a provider-focused team that understands the unique role of NPs in long-term care.

About the Role :

Provide primary care services to both short-term and long-term residents

Perform daily rounds, manage acute and chronic conditions, and coordinate care

Collaborate with facility nursing staff and interdisciplinary teams

Complete assessments, documentation, and communication with families and specialists

Ensure continuity of care across both locations

Requirements :

Active Nurse Practitioner license in New Jersey

At least 6 months of NP experience (ICU or SNF experience is a plus)

Strong clinical judgment and communication skills

Ability to work independently across two locations with reliable transportation

Passion for post-acute and geriatric care
