Note : This position requires approximately 9 months of in-office training at our Champaign, IL location before transitioning to remote work.

Your Role

As a Patient Services Nurse , you will play a vital role in coordinating the patient intake and triage process for our Primary Care Departments. Your expertise will ensure that patients receive the appropriate level of care and advice they need.

Key Responsibilities

• Assist patients via phone to assess the urgency of care needed, schedule appointments, and provide health information.
• Evaluate, assess, and document patient inquiries and concerns.
• Collaborate with Primary Care Center physicians and nursing staff to meet patient needs.
• Ensure accurate and complete documentation of care.
• Provide general health information and wellness education using approved resources.
• Maintain and support the completeness of patient electronic medical records, including medication reconciliation.
• Triage calls regarding emergent matters and provide education per provider-approved protocols.
• Assist primary care offices with patient lab results and prescription requests.
• Support the Medication Renewal Center by reviewing medication requests and disseminating provider education.
• Cross-train and provide float support to all Patient Contact Center Clinical teams as needed.

Compensation and Benefits

The compensation range for this position is $29 per hour - $49.88 per hour . This range reflects a good faith minimum and maximum for the role at the time of posting. Actual compensation will depend on various factors such as experience, qualifications, and location.
