Overview : \n\nPresbyterian is seeking a dedicated Outpatient Orthopedic Registered Nurse (RN) to join our clinic team. In this role, the RN provides specialized nursing care to patients with musculoskeletal conditions, including pre- and post-operative support for orthopedic procedures. Responsibilities include patient assessment, wound care, cast and splint application / removal, administering injections, coordinating imaging and referrals, and educating patients on treatment plans and recovery protocols. - assesses patient populations based on symptoms and medical history to identify care needs\n
- develops individualized care plans focused on prevention, using evidence-based practice guidelines\n
- collaborates with patients to update problem lists, medication records, and coordinate visit plans\n
- monitors patient follow-up issues such as missed appointments and non-compliance\n
- advocates for patients and serves as a communication link between inpatient and outpatient care managers\n
- documents care and interventions within scope of nursing practice\n
- mentors and supports colleagues by sharing knowledge and guiding professional development\n
- coordinates and delivers patient education using standard guidelines, validating understanding through verbal and written instruction\n
- evaluates nursing care delivery and seeks opportunities for process improvement with thorough and accurate work\n
- communicates with providers and community health professionals to ensure continuity of care\n
- supports Care Team Associates as an active member of the Primary Care Team\n
- performs telephone triage to assess symptoms, provide guidance, and determine appropriate care or referrals\n
- administers medications and vaccinations per protocol and documents accurately\n
- assists with procedures and treatments in clinic settings such as wound care, catheter management, and specimen collection\n
- reviews and processes medication refills and prior authorizations in collaboration with providers\n
- monitors and manages chronic conditions through routine follow-up, patient education, and care coordination\n
- ensures compliance with infection control standards and clinic safety protocols\n
- participates in quality improvement initiatives and data collection for performance metrics\n
