We value contributions from everyone involved in patient care.Your Role as a Hospice Nurse :
Conduct initial assessments of the patient’s physical and psycho-social status.Collaborate with the Interdisciplinary Group (IDG), patient / family, and attending physician to develop a personalized care plan.Provide education and interventions to effectively manage pain and other physical symptoms, while assessing the patient / family’s response.Ensure availability of necessary equipment, supplies, and medications.Adapt the IDG plan of care as the patient’s / family’s needs evolve.Document patient / family services accurately and in a timely manner.Participate regularly in IDG meetings to ensure cohesive care.What We’re Looking For :
Registered Nurse : Must hold an active, unrestricted license in the State of Michigan.Experience : Community health and a broad range of Medical-Surgical experience is preferred.Transportation : Reliable transportation, a valid state driver's license, and current car insurance are required.Join us in making a difference in the lives of those we serve.
