We are seeking a Medication Coordinator to serve as the bridge between our partner clinics and our specialty pharmacy. In this vital onsite role, you will build relationships with clinic staff, assist individuals with their medications and insurance, and promote Altruix's pharmacy services. Your work will ensure that individuals receive the support and medication management they need to thrive.

Key Responsibilities

• Act as an onsite liaison between the partner clinic and Altruix pharmacy
• Build and maintain relationships with agency staff and individuals receiving care
• Educate clinic staff and individuals on Altruix services and support offerings
• Recruit and enroll individuals using Altruix enrollment forms and insurance card collection
• Collect prescriptions and facilitate transmission to the pharmacy
• Ensure individual insurance information is current and accurately processed in the system
• Support individuals with medication-related questions by coordinating communication with pharmacists
• Assist with prior authorizations and patient assistance programs
• Monitor compliance by maintaining regular contact with individuals (at least monthly)
• Manage refills by monitoring reports and coordinating with prescribers
• When applicable, assist with medication deliveries (permitted states only)
• Check medication storage and expiration dates in both personal and house account medications
• Monitor for medication discrepancies, dose changes, and discontinuations
