Position : Clinical Pharmacist
Overview
We are seeking a Clinical Pharmacist to provide hands-on patient care, optimize medication therapy, and support providers in improving health outcomes. This role focuses on chronic disease management, patient-centered care, and collaboration with interdisciplinary teams. Care is delivered virtually, in-office, or at patients’ homes. Outpatient experience is highly sought after, and candidates with hospital, ambulatory, or health plan clinical experience are encouraged to apply.
Key Responsibilities
- Conduct comprehensive medication reviews and develop follow-up care plans with patients, providers, and care teams
- Assess and optimize medication therapy for safety, efficacy, and adherence
- Collaborate with interdisciplinary teams to set individualized therapy goals
- Educate patients on chronic disease management and appropriate medication use
- Deliver patient care under collaborative practice agreements, including refill approvals and disease-state management
- Participate in quality improvement initiatives and program development
- Serve as a trusted resource for medication information and pharmacy benefits
- Provide professional, timely, and empathetic communication with patients and colleagues
- Travel to multiple sites as needed (if applicable)
Qualifications & Requirements
Must-Have :
Pharm.D. degree or equivalent education in pharmacyActive, unrestricted pharmacy license in TX, or eligibility to obtain oneBilingual in Spanish (high-level)Clinical pharmacy background (no retail experience)Hands-on experience managing patient medication plansOpen to new graduates with 1–2 years of residency3–5 years of relevant clinical experience preferredNice-to-Have :
