Overview :
We are currently seeking an experienced and highly motivated Oncology Clinical Pharmacist to join a dynamic and collaborative healthcare team. In this role, you will be integral to the safe and effective treatment of cancer patients, working closely with oncologists, nurse practitioners, and pharmacy staff to optimize patient care throughout the continuum of oncology services.
Position : Oncology Clinical Pharmacist (Direct Hire)
Schedule : Full-Time, Day Shift (80 hours per pay period)
Start Date : ASAP
Key Responsibilities :
Collaborate with oncologists and multidisciplinary teams to ensure the safe and effective use of chemotherapy and supportive care medications in both inpatient and outpatient settings.
Serve as a clinical expert and resource for oncology pharmacy, contributing to treatment planning and decision-making.
Provide education and training to pharmacy staff, nursing teams, oncology fellows, and other healthcare professionals.
Participate in patient rounds with the oncology team to assess treatment efficacy, manage drug-related problems, and make evidence-based therapeutic remendations.
Conduct medication education sessions for patients starting oral oncolytics or receiving intravenous chemotherapy.
Review and maintain oncology care plans in the Electronic Health Record to ensure alignment with current guidelines and standards of care.
Identify and resolve medication-related issues using clinical judgment and patient data.
Precept pharmacy residents and students as part of professional development efforts.
Actively engage in Oncology Service Line initiatives to promote safety, efficacy, and cost-effective care.
Participate in continuous quality improvement initiatives, focusing on medication safety and outes.
Qualifications : Required :
Doctor of Pharmacy (PharmD) degree
Licensure as a Pharmacist
Board Certification in Oncology Pharmacy (BCOP)
Minimum of three (3) years of hospital pharmacy experience, with at least three (3) years of focused oncology practice or pletion of a PGY-2 Oncology Residency
Preferred :
pletion of a PGY-1 and PGY-2 ASHP-accredited residency program in Oncology
Five (5) or more years of experience in oncology pharmacy practice with BPS certification
