• Referral Program\nWe are currently seeking a Full Time RN Case Manager for the Bridgeport through East Haven Areas\n\nRequirements : \n
• Minimum 1 year Home Care Experience\n
• Comprehensive knowledge and skill in current nursing practice.\n
• Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery.\nKnowledge of the OASIS process.\n
• Knowledge of Community Resources.\n
• Ability to make meaningful observations, evaluations and reports.\n
• Ability to recognize the needs and concerns of patients, caregivers, and the multidisciplinary team to maintain therapeutic relationships.\n
• Ability to provide skilled nursing care based on scientific principles, the nursing process, basic behavioral concepts, and intelligent professional judgment.\n
• Ability to read, write and speak the English language in an understandable manner.\n
• Ability to bend, push, pull, move and / or lift\n
• Must possess a Nursing degree from an accredited College or University and be a Graduate of a CT approved RN program.\n
• Valid driver's license / Car insurance\n
• Active RN License in the State of CT\n
