job_description.job_card.job_descriptionProvide individualized support with activities of daily living (ADLs), including bathing, grooming, toileting, feeding, and mobility assistance. Assist individuals in participating in community activities, promoting inclusion and socialization. Foster independence and skill-building through everyday tasks and life coaching. Offer respectful behavioral redirection when necessary, following support plans and agency protocols. Monitor for changes in physical, mental, or emotional health and report observations promptly. Support medication administration and document compliance (if certified and applicable). Ensure a clean, safe, and supportive home or community-based environment. Accurately document daily activities, incident reports, and progress notes in accordance with Electronic Visit Verification (EVV) standards. Maintain confidentiality and compliance with HIPAA, Chapter 6100 regulations, and agency policies. Collaborate with supervisors, families, and team members to coordinate care and advocate for individual needs. High School Diploma or GED (Required) 1 -3 years of experience working in home care setting Valid Drivers License, clean driving record, and reliable transportation Ability to pass all background checks and drug screenings First Aid / CPR Certification EVV compliance knowledge or willingness to learn (training provided) Strong communication, empathy, patience, and decision-making skills
