Medford Treatment Center is seeking an Intake Coordinator to provide compassionate care to our clients.
$22.00hr-$26.00hr Depending on experience
Relocation Assistance Provided!
Our treatment center hours are : 5 : 00 a.m.- 1 : 00 p.m. M-F and 6 : 00 - 10 : 00 a.m. on rotating Saturdays.

Take incoming appointment request phone calls / Emails and outside transfers and referralsConduct Pre-Intake Screening on NEW clientsIf the person is using Private Insurance, ensure all items are complete before scheduling appointment, including copy of insurance card, collect co-pays, explain out of pocket fees, etc.Complete justification for not admitting, if applicableMaintain Pre-Admission Screening BinderObtain outside records before intake appointment when the client is a transferVerify insurance (OHP, DMAP, PSCS etc.)Schedule admission appointmentsFollow up on no shows and document in systemBuild client chart in computer systemSet up signature and pin, take client picture, complete consent to treat, obtain all ROIsComplete admission contract package Inform clients of ride share and reimbursement programs. Give business card, ride sheets, and booklet, if applicableGive client a building tour and review the admit process and what to expect at time of admissionsKnowledge and skills required for the position are :
High school diploma or equivalent and have one (1) year experience as a general office clerkAccurate data entry skills with knowledge of basic computingDemonstrate basic knowledge and skill in the use of standard office equipmentValid driver's licenseM eet Oregon requirements for employment in an opioid treatment programWe follow federal guidelines regarding prohibited substances, even those legal at the state level.Visit our website at to learn more about our mission.
