Overview
Senior Community Care Michigan (PACE) is part of Volunteers of America National Services, the Housing and Healthcare affiliate of the Volunteers of America parent organization. Position : Social Worker Supervisor (LMSW Required) at the SCCMI PACE clinic. Pay : $77,400-$88,500. Location : Lansing, MI.
Responsibilities
- Direct supervision of social workers, behavioral health specialists, chaplain and social work case managers.
- Apply advanced social work knowledge and skills in participant screening, case management, counseling and referrals under the supervision of the Center Director.
- Coordinate with interdisciplinary teams to develop plans of care and ensure comprehensive services for program participants.
Required Qualifications
Master's Degree from an accredited school of social work.LMSW license / eligibility as applicable in the state of employment.Minimum of two (2) years' experience working with frail or elderly population.Valid driver's license, proof of insurance and means of transportation.Medical clearance for communicable diseases and up-to-date immunizations prior to direct participant contact.Preferred Qualifications
Experience on a multidisciplinary team in hospital, nursing home or community-based settings.Ability to conduct monthly department meetings and oversee social work, behavioral health and spiritual care.Skills to evaluate participants' changing needs and adjust programs accordingly.Experience supervising direct reports and providing clear responsibilities and accountability.Oversight of elder abuse training in coordination with Quality and Compliance teams.Capability to manage participant panels to support equity and efficiency.Ability to conduct initial in-person social work assessments and coordinate with the Interdisciplinary Team to develop a comprehensive plan of care.Benefits
