Job Title : Social Work Supervisor
Reports to : Project Choices Director
Work Location : Lansing, MI
Work Schedule : Monday - Friday, 8 : 00am to 5 : 00pm, 40 hours per week.
Compensation : 34.97 / hr. plus comprehensive benefits package including 100% employer paid pension, generous time off and 14 paid holidays.
General Responsibilities :
Responsible for the overall supervision and administration of the Supports Coordinator component in the Project Choices Program, a long-term care system designed to serve the elderly and disabled who are at-risk of entering a nursing facility or who are residing in a nursing facility. Promotes and practices the principle of Person-Centered Thinking. Directs, monitors, and evaluates Supports Coordinators in the provision of participant services. Participates in the development and implementation of the quality systems designed to measure and evaluate quality indicators and outcomes.

- Using the principles of Person-Centered Planning, consults, evaluates and directs the Supports Coordinators and Case Coordinators in all work-related matters.
- Oversees Supports Coordinators and Case Coordinators including, but not limited to, monitoring performance, reviewing participant charts, reviewing and authorizing Care Plans, approving the purchasing of services, and resolving billing issues.
- Helps resolve disputes between participants, service providers and staff.
- Monitors Supports Coordinators and Case Coordinators COMPASS dashboard, analyzes performance and provides feedback.
- Review of Nursing Facility Level of Care Determination (NFLOCD) and comprehensive assessment in COMPASS to ensure consistency and forwarding or entering into CHAMPS.
- Monitors Critical Incident Portal, reviewing entries, assigning follow up and providing guidance as needed, and approving for MDHHS.
- Member of the Quality Assurance Team and Process Improvement Team.
- Oversees 24 hour on-call services and acts as back up on a rotating basis
- Provides input to the Quality Manager for in-service training and staff development activities as required.
- Conducts meetings on a variety of topics and assists with internal & external presentations as needed; co-leads monthly case conference and oversees hospitalization meeting lead by Lead Supports Coordinators.
- Works closely with the Nursing Supervisor, providing guidance, direction and supervision in their absence.
- Assists with quality activities to prepare for and respond to the Clinical Quality Assurance Review (CQAR) and Administrative Quality Assurance Review (AQAR).
- In conjunction with the Nursing Supervisor, supervises and trains Supports Coordinators and Administrative Assistants. Makes recommendations for employment, terminations, corrective action, and conducts performance appraisals to maintain Agency standards. Trains and supervises students.
- Is an active member of the agency Compliance Team.

Must be a licensed Social Worker with a current Michigan MSW license in good standing. Master's degree in social work is required.Must have five years of relevant experience. Experience with the elderly and / or people with disabilities is preferred.Previous supervisory experience is preferred.Must have a valid Michigan's Driver's license in good standing (less than 4 points).Working Conditions :
The work environment is a typical office setting. Safe work practices in regard to office equipment, avoiding trips / falls and fire regulations are required. Occasionally, outside visits to residential homes are necessary and exposure to varying environments, temperatures and noise levels increase.Physical mobility is required for sitting, walking, standing, bending, and lifting / holding / carrying objects of up to 20 pounds. Ability to enter and access information using a computer. Must be able to drive.
