General Description :
Healthy Texas Women (HTW) Patient Navigators will support the expansion of PHNTX’s HTW program. This position plays a vital role in helping individuals access healthcare services, overcome barriers to care, and remain engaged in continuous, comprehensive healthcare. These navigators will assist with enrollment in HTW and other financial assistance programs, provide education and referrals, and collaborate with community organizations to increase awareness and access. Responsibilities Specific Responsibilities of the Job :
Assist patients with enrollment in HTW and other financial assistance programs, and schedule appointments for patients and their children.Screen for eligibility and ensure timely and accurate enrollment.Provide culturally competent, bilingual support throughout the enrollment process.Coordinate referrals to wraparound service including behavioral health, reproductive health, social services, and specialty care.Ensure seamless transitions between services within PHNTX and through community partners.Build and maintain partnerships with trusted community organizations.Represent PHNTX at community events to increase awareness of HTW services.Conduct targeted outreach to women who are eligible but not yet enrolled in HTW.Educate patients on preventive care, routine screenings, and the importance of continuity of care.Engage in ongoing training related to HTW program guidelines, eligibility criteria, and patient engagement strategies.Connect patients to internal and external services and resources.Be able to work evenings and weekends and be flexible with work hours.Manage small caseload and complete needs assessments, follow up on access to external resources and ensure adherence with internal appointments.Required Skills Required Knowledge, Skills, and Abilities :
Strong interpersonal and communication skills with the ability to build trust and put clients at ease during sensitive conversations.Demonstrated cultural competence and ability to work professionally with individuals from diverse racial, ethnic, and socio-economic backgrounds.Knowledge of public health programs (HTW, Medicaid, CHIP) and community resources, including medication assistance programs.Experience in ambulatory care settings and familiarity with medical and psychosocial complexities.Proficiency in Microsoft Office Suite (Excel, Word, Outlook, Teams) and experience with electronic medical records (EMR) preferred.Ability to work independently, manage multiple priorities in a fast-paced environment, and make patient-centered decisions with minimal supervision.Strong advocacy skills and the ability to collaborate effectively with internal teams and external service providers.Must maintain valid driver’s license and vehicle insurance at all timesMust have reliable transportationEducation and Experience :
High school diploma or GED required; associate or bachelor’s degree in social work, public health, or a healthcare-related field preferred.1–2 years of experience in patient navigation, case management, community health work, or related field.Bilingual in English and Spanish preferred.
