Job Title : Registered Nurse (RN) – Home Health | 13-Week Contract | Days | $60 / hr\n\nLocation : Columbia, TN\nContract Duration : 13 weeks\nShift : 8-hour Day Shifts\nPay Rate : $60 per hour\n\nAbout the Role : \nWe are seeking an experienced and compassionate Registered Nurse (RN) to join our Home Health team in Columbia, TN for a 13-week contract. The RN will provide skilled nursing care to adult patients in their homes, ensuring high-quality, patient-centered care and accurate documentation in accordance with agency and regulatory guidelines.\n\nThis is an excellent opportunity for an RN who enjoys independent, field-based work and values one-on-one patient care.\n\nResponsibilities : \n

• Conduct comprehensive patient assessments and develop individualized care plans.\n
• Provide case management and coordinate interdisciplinary care to meet patient goals.\n
• Complete all required documentation including : \n

• Discharge (including for safety or noncompliance)\n
• Recertification\n
• Resumption of Care\n
• Routine Visit\n
• Significant Change in Condition\n

• Transfer Documentation\n
• Monitor, evaluate, and document patient progress, communicating changes promptly to the clinical team.\n
• Educate patients and families on disease management, medication administration, and home safety.\n
• Manage and administer medications safely and accurately.\n
• Provide wound care, IV therapy, and specialized nursing interventions as required.\n\nClinical Expertise and Skills : \n\nWork Settings : \n

• Clinical assessment documentation and coordination of services\n
• Palliative symptom management\n
• Nutritional assessments and GI management, including : \n
• Colostomy / Ileostomy care\n
• Feeding tube management (G-tube, NG tube)\n
• G-tube site care\n\nIntegumentary / Wound Care : \n
• Drain management\n
• Pressure ulcer prevention and staging\n
• Wound care and Wound VAC management\n\nMedication and Pain Management : \n
• Medication administration and reconciliation\n
• Pain assessment and management using verbal and nonverbal pain scales\n\nRespiratory Care : \n
• Nebulizer education and use\n
• Oxygen safety and administration\n
• Tracheostomy care\n
• Ventilator management and maintenance\n\nQualifications : \n
• Active Registered Nurse (RN) license in the state of Tennessee.\n
• Minimum of 2 year of recent Home Health experience required.\n
• Current BLS certification.\n
• Knowledge of OASIS and home health documentation standards preferred.\n
• Strong clinical assessment and organizational skills.\n
• Reliable transportation and valid driver's license.\n
• Ability to work independently and manage a diverse caseload.\n\nJob Details : \n
