What you’ll do :
The Transitional Age Youth Case Manager will support youth in developing and maintaining important life-skills as they transition from the children’s system of care to the adult system of care. This position provides direct service crisis intervention, intake, assessments, client support, transportation, and education to individuals and families, independently of, or in conjunction with, other in-home client services. This position will, also, be responsible for making referrals to collaborating partners and other community services, as client needs indicate. Ensure delivery of quality service including clinical care and documentation while maintaining all client records in accordance with agency policies and procedures.
