Job Title : Direct Support Professional – Per Diem I
Location : Rochester, NY 14624 – Various Locations
Department : Relief
Reports To : Workforce Management
Employment Type : Per Diem - Minimum of 16 Hours every 2 weeks required
Starting Wage : $17.00 / hr (Day Services) | $18.00 / hr (Residential)

Your Core Responsibilities :
Provide respectful personal care including bathing, grooming, toileting, dressing, and meal support.Support individual goals, independence, and skill development while promoting community inclusion.Implement individualized plans including IPOP, SAP, BSP, and PONS.Maintain accurate documentation in the Electronic Health Record (EHR).Ensure health, safety, dignity, and well-being at all times, reporting concerns immediately.Foster positive communication with individuals, families, co-workers, and supervisors.Drive agency vehicles to transport individuals to appointments, programs, or community outings.What You Bring :
Compassion, patience, dependability, and respect for others.Ability to follow individualized care plans and maintain required supervision levels.Basic computer skills for documentation and communication.Strong communication and problem-solving abilities.Flexibility to work at various sites and shifts as needed.Ability to lift / move up to 35 pounds and perform physical care responsibilities.Required : Maintain certifications in SCIP-R and First Aid / CPR (training provided).Must pass required background checks and clearances (SCR, fingerprinting, SEL, MHL).Preferred : Prior experience in human services or direct care.Our Mission & Culture :
