Community Outreach Specialist We are seeking a Full-Time Community Outreach Specialist to become part of our fast-growing team! In this role, you will provide in-person outreach to patients who may have been difficult to reach by phone, text, or mail.
Your work will help reconnect members with their healthcare providers, support care coordination, and improve access to services — ultimately helping close gaps in care. Responsibilities
- Conduct in-person outreach attempts at members’ last-known addresses, following approved safety protocols and schedule.
- Knock on doors professionally, engage members using approved scripts, and potentially leave door hangers with callback information if no one answers.
- Confirm address accuracy through observation or speaking with neighbors / property staff using HIPAA-compliant language.
- When a member is located, assess the patient’s environment for safety and other factors, administer a questionnaire to identify their need for social supports, and remind them of healthcare services for which they are due.
- Document all outreach attempts, outcomes, observations, and questionnaire responses in the company system.
- Communicate barriers (e.g., building access issues, unsafe conditions) to supervisors and recommend solutions.
- Represent the organization professionally and respectfully in the community. Qualifications and Requirements
- High school diploma or equivalent required
- Prior outreach, canvassing, social services, or customer-facing experience required.
- Community health worker training or experience in the healthcare industry preferred.
- Strong communication skills, cultural competence, and ability to build trust quickly.
- Must have a valid driver’s license and reliable transportation that you are willing to use for work.
- Comfortable working independently in the field and navigating neighborhoods within the Chicago metropolitan area or other counties throughout Illinois depending upon assignment
- Willingness to work flexible hours, including some evenings and weekends.
- Ability to thrive in a fast-paced environment while meeting documentation and performance requirements.
- Must pass a background check and drug screening. Salary and Benefits The salary for this position is $22 / hr.
