Location : Avera at Home Sioux Falls\n\nWorker Type : Regular\n\nWork Shift : Primarily days with possible evenings, weekends, or holidays (United States of America)\n\nPay Range : $30.50 - $45.75 per hour, dependent on experience.\n\nPosition Highlights : \n

  • Work schedule : Monday-Friday, 8 : 00 am-4 : 30 pm; 72 hours per 2 weeks.\n
  • Weekend rotation : 1 weekend every 7 weeks.\n
  • Holiday rotation : 1 holiday every 2 years.\n
  • Call time involvement : 4-5 call shifts per 7-week schedule.\n\nMay be eligible for a $10,000 Sign-on Bonus\n\nStudent Loan Repayment : This Registered Nurse (RN) position may qualify for Avera's Student Loan Repayment Program, which can help you pay off your student loans more quickly. Here, your contributions truly matter.\n\nA Brief Overview : \n\nAs a Registered Nurse in Home Health Care, you will be responsible for care planning, coordinating services, and educating interdisciplinary teams on patient care management. You will collaborate with physicians and the care team to develop comprehensive patient-centered care plans to ensure exceptional outcomes.\n\nKey Responsibilities : \n
  • Conduct comprehensive patient assessments addressing physical, psychosocial, emotional, spiritual, and bereavement needs.\n
  • Utilize skilled nursing assessments during Start of Care visits, post-hospital evaluations, and supervisory visits.\n
  • Partner with the physician and interdisciplinary team to create individualized plans of care.\n
  • Implement clinical protocols efficiently during admissions based on patient diagnoses and outcomes.\n
  • Evaluate requests for changes in plans of care and assess the need for updates.\n
  • Provide thorough education to patients and caregivers on care alternatives, disease management, and community resources.\n\nEssential Qualifications : \n
  • Must hold a Registered Nurse (RN) license upon hire.\n
  • Visual acuity and effective communication abilities are necessary to perform essential job functions.\n\nPreferred Qualifications : \n
  • 1-3 years of experience in home health or hospice settings.\n\nExpectations and Standards : \n
