Title : EHE Non-Medical and Employment Case Manager
Summary
The EHE Non-Medical and Employment Case Manager provides individualized, client-centered support to participants enrolled in the Ryan White Part A and Ending the HIV Epidemic (EHE) programs. This position is responsible for assessing client needs, removing barriers to care, and linking participants to essential medical, social, and employment-related services to promote self-sufficiency, retention in care, and improved health outcomes.

Required Education and Experience
Bachelor’s degree in human services, Social Work, or a related field required. Valid driver’s license, excellent writing and computer skills required. Valid driver’s license and reliable transportation. Strong written, organizational, and computer skills. Experience providing employment support or case management preferred.
Physical Demands
While performing the duties of this job, the employee is frequently required to stand; walk; sit, use hands, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and / or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
