Job Title : Nurse Practitioner (NP)

Location :  Klamath Falls, OR

Salary Range :  $115,000 – $155,000 annually (Negotiable)

Schedule :  Monday–Friday, 8 : 00 AM – 5 : 00 PM (No on-call or after-hours)

Responsibilities

• Provide facility-based care in Skilled Nursing, Assisted Living, and Memory Care settings
• Collaborate with interdisciplinary teams including physicians, therapists, and social workers
• Conduct patient assessments, develop care plans, and monitor progress
• Build long-term relationships with patients and families
• Document care accurately and timely
