    Position Summary

    This is a full-time position grant-funded by Philly CEAL, University of Pennsylvania, National Institute of Health and CEO. The Community Health Worker Manager will manage up to 2 CHW Supervisors, who supervise up to 8 CHWs in promoting positive health outcomes through care referrals and information sharing, while also organizing their local community around the social determinants of health to improve access to health initiatives. This position is part of the Place-Based Initiatives unit and is responsible for identifying the best practices to be applied to the broader CEO.

    Philly CEAL is a city-wide collaborative effort aimed at improving the health and well-being of Philadelphia communities. Philly CEAL is part of the NIH CEAL research network, which is designed to identify promising engagement and outreach practices that communicate trustworthy, science-based information to communities experiencing health disparities. The aims of Philly CEAL include to :

  • Implement and evaluate the real-world effectiveness of a group-delivered Community Health Worker (CHW) program to improve chronic disease self-management.
  • Promote collaborations between multiple sectors to promote new opportunities to improve prevention and care service delivery.
  • Address the social determinants of health that drive health disparities and inequities in chronic disease self-management.
  • Essential Functions

    This position requires a mix of in-office and community-based duties, including evening and weekend hours. The Manager will complete and / or oversee these standard essential functions :

  • Program Leadership and Oversight : Provide leadership for all aspects of the CHW program, including strategic planning, program development, and implementation. Ensure that CHW initiatives are effectively integrated into the larger organizational framework and meet compliance and performance standards.
  • Team Management and Development : Supervise CHW Supervisors, fostering a collaborative and productive work environment. Provide guidance, training, and support to CHW Supervisors and their teams of Community Health Workers to ensure high performance and professional growth.
  • Human Resources Oversight : In collaboration with CEO HR and Senior Leadership, oversee some HR functions for the CHW Program, including staff hiring, and overseeing structured onboarding and offboarding processes to ensure compliance and effective staff integration. This also includes taking accountability measures with staff not limited to leading disciplinary procedures on the behalf of CHW supervisors
  • Partnership and Stakeholder Engagement : Develop and maintain strong relationships with key healthcare partners, stakeholders, and community leaders. Represent the CHW team in various forums and engage with stakeholders to enhance program outreach and impact. Work with CHW Supervisors to incorporate action plans and resources related to addressing chronic disease prevention and management, as well as needs relating to social determinates of health (SDOH)
  • Financial and Resource Management : In partnership with CEO Fiscal, Contracts, and Operations teams, oversee the allocation and utilization of resources within the CHW program, including budget management, staffing, and logistical support. Ensure that resources are used efficiently and effectively to meet program goals.
  • Performance Monitoring and Evaluation : Implement and maintain systems for monitoring and evaluating program effectiveness. Utilize data and feedback to improve program operations and outcomes. Prepare and present regular reports on program progress to senior management and external stakeholders.
  • Risk Management : Identify potential risks to the program and implement appropriate mitigation strategies. Ensure compliance with all relevant regulations and policies.
  • As it relates to the Philly CEAL program, the CHW Program Manager is responsible for the quality control and implementation of all procedures related the study, including the achievement of the identified milestones in the CEAL application. All above responsibilities also apply to CEAL program oversight.
  • Perform any additional duties as described by the Community Health Worker Program or Office of Community Empowerment and Opportunity (CEO) related to the goals and activities of the Office and Program.
  • Competencies, Knowledge, Skills and Abilities

    Candidates will have most of the following competencies, knowledge, skills and abilities :

  • Must have a valid driver’s license
  • The ability to transport materials routinely moves equipment or supplies weighing up to 30 pounds.
  • High school diploma or GED required.
  • A passion for serving their community.
  • Desire to develop and practice community organizing skills.
  • Knowledge of Philadelphia’s communities and the ability to effectively engage individuals with diverse backgrounds
  • Demonstrates understanding of Social Determinants of Health (SDOH) and their connection to chronic disease prevention and management
  • Proven ability to build relationships, collaborate, and engage in person-centered support.
  • Strong oral communication skills and experience presenting in community or stakeholder meetings.
  • Strength in troubleshooting and developing creative solutions.
  • Experience in data collection, analysis, and reporting as part of program planning or evaluation.
  • Facilitation experience in meetings or workshops focused on resource sharing, outreach, or wellness education.
  • Knowledge and use of resource and referral systems
  • Qualifications

    To be successful in this role, candidates should have experience with most of the bullet points below. We also welcome candidates that have non-professional experience in these areas. For example, lived or volunteer experience can be just as valuable as professional experience. Our primary qualifications for this position include :
