A company is looking for a Wellness Coordinator who provides care coordination and administrative support for members with behavioral health and substance abuse needs.
Key Responsibilities
Coordinate member care through inbound and outbound communication, screening for gaps in care
Provide administrative support to the Behavioral Health Case Management department, including documentation and referral assignments
Educate members on behavioral health benefits and connect them with appropriate resources
Required Qualifications
High School Diploma / GED (or higher)
2+ years of managed care or call center experience
1+ years of computer proficiency experience
