Job Summary
We are seeking a compassionate and dedicated Holistic Community-Based Nurse with training in Lifestyle Medicine and clinical research to join our Community Health Outreach team. This role is split between promoting wellness, preventing chronic disease, and improving health outcomes by integrating evidence-based lifestyle interventions with community-based nursing practices and coordinating all clinical research activities per study protocol. The ideal candidate is passionate about culturally competent care and empowering individuals and families to make sustainable health behavior changes.
Qualifications
Current licensure by the New Jersey Board of Nursing as a Registered Professional Nurse
Current and valid driver’s license, registration, inspection sticker, and appropriate level of insurance coverage required, to include collision to perform the essential duties of the position.
Certification or formal training in Lifestyle Medicine (e.g., ACLM).
Certification in CITI Clinical Research Coordinator course or other CITI training.
Minimum 2 years of experience in community health, public health nursing, or related field.
Minimum 2 years of experience in clinical research or a related healthcare setting.
Strong organizational and multitasking abilities.
Strong communication, education, and motivational interviewing skills.
High attention to detail and ability to follow procedures accurately.
Experience working with diverse, underserved, and vulnerable populations.
Ability to work independently and as part of a team in a dynamic community setting.
Bilingual skills are a plus.
Education
Graduate from an accredited School of Nursing
BSN preferred

Essential Functions
Conduct comprehensive community health assessments to identify population health needs.
Collaborates with the Program Director and participates in program development, research and evaluation.
Manages the day-to-day activities of clinical trials, including recruiting and screening participants, obtaining informed consent, collecting and documenting data, and ensuring all study activities comply with protocols and regulatory guidelines.
Design and implement lifestyle medicine interventions focused on nutrition, physical activity, sleep, stress management, substance use, and social connection.
Provide individual and group education sessions in community settings (e.g., schools, faith-based organizations, shelters).
Develop culturally tailored outreach strategies that address social determinants of health.
Serve as a liaison between community members and local health resources and services.
Advocate for policies and programs that support holistic health and community well-being.
