Licensed Practical Nurse (LPN) - Long-Term Nursing and Rehabilitation
Overview :
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our long-term nursing and rehabilitation team. The ideal candidate will provide high-quality patient care to residents in a comfortable and therapeutic environment, assisting individuals in their recovery and daily living activities.
Key Responsibilities :
- Patient Care : Deliver direct patient care under the supervision of Registered Nurses (RNs) and physicians, including administering medications, monitoring vital signs, and assisting with daily living activities such as bathing, dressing, and feeding.
- Rehabilitation Support : Collaborate with rehabilitation therapists to implement and support individualized care plans, encouraging residents to reach their maximum level of function.
- Documentation : Maintain accurate and up-to-date patient records, including assessments, interventions, and progress notes, ensuring compliance with facility policies and regulatory standards.
- Team Collaboration : Work closely with nursing staff, therapists, and other healthcare professionals to provide comprehensive care and develop a supportive environment for residents.
- Patient Education : Educate residents and their families about health conditions, treatment plans, medications, and rehabilitative exercises.
- Infection Control : Adhere to infection control protocols to ensure the safety and well-being of residents and staff.
- Emergency Response : Respond promptly to emergency situations, implementing appropriate intervention measures as needed.
- Facility Policies : Comply with all facility policies, procedures, and regulations governing nursing practice and patient care.
Qualifications :
Licensure : Current and valid LPN license in the state of practice.Experience : Previous experience in long-term care, rehabilitation, or similar healthcare 1 or more years.Education : Graduate of an accredited practical nursing program.Skills :Excellent communication and interpersonal skills.Strong organizational and time-management abilities.Proficient in electronic medical records (EMR) and basic computer skills.Compassionate and patient-centered approach to care.Physical Requirements :
Ability to lift and assist patients as necessary.Stand, walk, and move for extended periods.Bend, stretch, and reach as required in patient care tasks.Work Environment :
Fast-paced nursing and rehabilitation facility.May require shift work, including nights and weekends. 12 Hour shifts.
