A thriving healthcare organization is seeking a dedicated Social Worker for a 2-month temporary assignment to support Case Management and Inpatient Psychiatric teams. The role focuses on providing essential psychosocial support to patients and families, helping them navigate illness, lifestyle adjustments, and transitions in care.
Responsibilities :
- Conduct assessments of patients and families, develop individualized care plans, and implement interventions with a multidisciplinary team.
- Assist patients and families in accessing financial, social, and legal resources.
- Facilitate meetings to address psychosocial and economic concerns impacting care and discharge planning.
- Support placement into long-term care facilities, including reviewing financial resources and providing family education.
- Offer counseling for patients and families coping with illness, lifestyle changes, or bereavement.
- Identify and address environmental or social barriers affecting patient outcomes.
- Coordinate interventions in cases of suspected or confirmed abuse or neglect.
- Lead legal processes such as guardianship, competency evaluations, and adoption matters.
- Conduct support groups and actively participate in team meetings and rounds.
The Ideal Candidate :
Master’s degree in Social Work (preferred) or Counseling.Minimum 2 years post-master’s experience in a medical setting.LCSW license in Connecticut preferred.Strong communication, negotiation, and critical thinking skills.Effective interpersonal skills and ability to collaborate within a team.Basic computer skills and familiarity with healthcare systems.Knowledge of healthcare reimbursement.Comfortable in a fast-paced, patient-centered environment.Click APPLY Now!
