Registered Nurse (RN) Coordinator - Cardiac Post Transplant

• Department : Cardiac Post Transplant Services
• Schedule : Full-time, Day Shift, 40 hours per week
• Hospital : Ascension Saint Thomas West
• Location : Nashville, TN
• Coordinates clinical and administrative processes for patients through all phases of the transplant / mechanical circulatory support (MCS) implant continuum to include pre-transplant / implant evaluation, wait list management, transplantation / implantation and post-transplantation / implantation follow-up care.
• Educates patients, families and staff regarding transplantation / MCS implantation.
• Collaborates with physicians and other program staff in evaluating and managing patients.
• Completes data capture, analysis and reporting and other administrative functions required for transplant / MCS program operations.
• Actively participates in the quality agenda for transplant services as outlined by the United Network of Organ Sharing (UNOS) and Centers for Medicare and Medicaid Services (CMS).
• Collects and maintains time study data for purposes of reporting pre and post transplant time.
