What You'll Do :

• ️ Assist families with basic needs and resources to promote self-sufficiency
• ️ Develop service plans with measurable goals
• ️ Provide in-home services when appropriate
• ️ Visit each client at least once per week
• ️ Facilitate networking and linkages with other community services
• ️ Maintain accurate case notes and records
• ️ Complete referral forms, social histories, and closure reports within designated timeframes
• ️ Participate in team meetings and staff reviews to evaluate client progress
• ️ Follow up on referrals and coordinate necessary services

What We're Looking For : 🎓 Education : 2 years of college at an accredited institution ( Associate's Degree or higher preferred ) 📌 Experience : Knowledge of trauma-informed care and willingness to engage in ongoing training 🚗 Requirements :
