Job Schedule : Full-time

Weekly Hours : 35.00

Salary Range : $25.50 - $38.50

Department Details

Enjoy daytime hours from Monday to Friday, working 35 hours each week in a supportive and collaborative environment.

Job Summary

As a vital member of our interdisciplinary team, you will :

• Provide supportive services and counseling on healthcare and home care programs.
• Assist patients, clients, residents, and their families in navigating social, emotional, and economic challenges.
• Coordinate healthcare programs, ensuring seamless communication among all parties involved.
• Respond to suspected abuse, neglect, or violence in accordance with ethical guidelines and state laws.
• Develop tailored care plans by connecting families with essential resources.
• Participate in care conferences and advocate for patients’ needs.
• Utilize your critical thinking and problem-solving skills to make informed decisions.

Depending on the department, you may also provide social services for donors and transplant recipients.
