Collaborate closely with a multidisciplinary care team to support safe, efficient transitions across the care continuum.

Apply evidence-based criteria to promote high-quality, cost-effective care.

Take the lead on discharge planning, utilization review, and coordination of services that support patient recovery and independence.

Act as a vital link between patients and the services they needconnecting them to the resources that help them thrive both in and outside the hospital.

Requirements

Education :

Associates Degree in Nursing with ongoing efforts to obtain Bachelors Degree.

Experience :

Minimum of 3 years clinical experience required. Previous experience in discharge planning, utilization management, knowledge of community resources, assessment skills and clinical and social factors that affect patients functional status at discharge preferred.

License :

Currently licensed as an RN in the State of Florida.

Certifications :

Case Management certification in : ACM, CCM, NACCM, or CMCN required within 5 yrs. employment.

Other :

Communication and interpersonal skills, basic computer, analytical, grammar and spelling skills. Ability to effectively multi-task and prioritize responsibilities.
