Key Responsibilities Contact patients who are preparing to leave or have recently left a care facility Introduce yourself, explain our pharmacy service, and guide them through their next medication order Review patient profiles received from the facility to ensure accuracy Arrange and confirm delivery of the second fill (upcoming order) Keep patients informed and answer any questions regarding their medications or order Coordinate with pharmacy teams to ensure timely processing and delivery Manage caller queue tasks and assist with handoff to Personal Care Coordinators Communicate with other departments (pharmacy, billing, insurance) when needed Document all interactions accurately and follow up as necessary Qualifications 1–2 years of experience in a customer service or patient-facing role
