Accountable for the oversight of the Hospice Quality Reporting Program and necessary submissions.Assists to educate agency on Hospice Quality Reporting Program updates and upcoming changes to ensure agency meets all related regulatory requirements.Promotes continuous agency quality improvement through development and execution of projects.Performs clinical audits to ensure highest quality care expectations being met and clinical compliance with hospice regulations.Assists to track quality measurements, including but not limited to, adverse events, infections, HQRP measures, and other Performance Improvement Plan data tracking.Participates in Hospice’s Internal Auditing Program. Graduate of accredited school of nursing for Registered Nurse; registered and licensed in the State of Minnesota Excellent verbal and written communication skills Strong organization skills-able to manage projects from initiation to implementation. Strong leadership skills with ability to provide clear direction and education on quality projects. Competency with Microsoft Office programs, Electronic Medical Record Systems, CMS programs, and database management and analysis. Knowledge on the Hospice Quality Reporting Program, hospice regulations; Federal, State and Medicare COP’s Job Duties : Responsibilities Monitors the measurement, implementation, evaluation of the following quality measures, including but not limited to; Hospice Care Compare, CAHPS, HCI, HOPE, and PEPPER. Compiles, tracks, and analyzes Hospice Quality Reporting Program and other quality data to create relevant agency goals for improvement in quality outcomes Participates in QAPI meetings, assists in agenda creation, minutes, and maintenance of records Reviews HOPE Data in accordance with regulations and completes timely submission through iQIES platform and coordinates care team schedule to ensure HOPE Admission, HUV 1, HUV 2 and SFV scheduled and completed in compliance with CMS HQRP Regulations. Provides direction and coordination of quality improvement activities utilizing input from the team, current data trends and other methodologies Reports quality data concerns to direct supervisor in a timely fashion Participates in Medicare and State audits including, but not limited to RAC, TPE, and other ADRs. Participates in the agency’s internal auditing program and performs clinical audits to ensure quality documentation in accordance with CMS regulations Participates in relevant teleconferences Investigates negative CAHPS complaints and propose action plans to improve quality Other duties as assigned Safety and Compliance : Is aware of safety issues and strives to accomplish all responsibilities in a safe manner.
