Pay and summary
Base pay range : $19.85 / hr - $28.15 / hr
Summary : Perform diversified job duties to support assigned adult individuals with intellectual / developmental disabilities in their group supported employment situations. Duties include transportation to / from individuals' residences and through the workday using Agency owned / leased vehicles or personal vehicle where applicable.
Essential duties and responsibilities
- Work as part of the interdisciplinary team to assist individuals in identifying and making choices about their social, vocational, and behavioral goals. Maximize participation in all phases of program development and implementation. Provide training and support within specified timeframes to help individuals achieve their goals.
- Train individuals to perform essential work tasks in a workplace-appropriate and productive manner.
- Provide ongoing support to ensure enhanced job performance and satisfaction.
- Act as day-to-day liaison with employers and co-workers to encourage integration into the workforce.
- Ensure employer production and quality control needs are met; may complete actual work tasks as appropriate.
- Utilize approved behavioral management techniques to improve inclusion in community work settings.
- Assess physical environments of service locations and transportation routes to identify hazards and report safety concerns.
- Provide transportation services to individuals daily, including to / from home as described in program materials.
- Provide required documentation to meet agency, state, federal, and interdisciplinary standards, including progress toward goals and barriers.
- Use computer to complete service documentation in the program data management system (goal tracking, activity records, incident reporting, behavior tracking, health monitoring).
- Provide disability awareness information to advocate for safe and accessible community resources.
- Assist individuals with personal care needs as described in the Physical Demands section of the job description.
- Provide required, individual transportation using appropriate vehicles, including pre / post-trip inspections, mileage documentation, and reporting safety concerns to the Transportation Coordinator.
- Secure passengers' wheelchairs with restraints, ensure seat belts and safety equipment are used, communicate with base via radio, and service vehicles as needed.
- Keep records of trips and document passenger behavior as required by program procedures.
Qualifications
Education and / or experience : High School Diploma or GED plus 1-3 years related experience OR bachelor's degree in a related field.Language skills : Ability to read and interpret documents, write professional reports, and clearly communicate verbally and in writing.Mathematical skills : Ability to perform basic arithmetic operations and compute rates, ratios, and percentages.Reasoning ability : Ability to solve practical problems, interpret instructions, observe behavior, and advocate for workers.Certificates, licenses, registrations : Valid Connecticut driver's license; ability to secure Public Service License within 90 days of hire; current vehicle registration and insurance; may require use of personal vehicle.Other skills and abilities : Ability to obtain PMT, CPR and First Aid certifications.Physical Demands : Detailed physical requirements reflecting standing, lifting, movement, and ability to assist individuals with personal care needs as described in the job description.Work environment
