CANS Assessor (Erie)

We're Hiring : Adolescence IDD Assessor!

We are seeking a   CANS Assessors to support a statewide initiative aimed at evaluating and enhancing services for youth (ages 21 and under) with IDD. In this role, you’ll conduct in-person assessments using the Child and Adolescent Needs and Strengths (CANS) tool in home and community-based settings.

What You'll Do :

• Conduct in-person CANS assessments in homes and community settings.
• Travel throughout the NYC region to meet with youth, families, and caregivers.
• Use a person-centered approach to evaluate strengths, needs, and developmental concerns.
• Gather information from caregivers, service providers, and relevant records to complete the assessment.
• Accurately document findings in state-mandated systems while meeting compliance standards.
• Coordinate language access services as needed.
• Maintain productivity and quality standards defined by the program.

What We're Looking For :

Education :   Bachelor’s degree in a human services field (required)
